Updated USPS Form 1583 (April 2023)

USPS recently released an updated version of Form 1583. The Postal Service updated the form to align with the recent changes in their Domestic Mail Manual. These changes aim to better collect and manage information from the form using an electronic database (CMRA Customer Registration Database).

The new form makes it easier for Commercial Mail Receiving Agencies to process the application and information of their PMB customers digitally.

What Do I Need to Complete the USPS Form 1583?

When you open a mailbox, two current forms of ID are required.

One must be a photo ID. The other must include your physical address.

Acceptable Photo ID: ​

  • U.S. State/Territory/Tribal Driver’s or Nondriver’s ID Card

  • Passport

  • Uniformed Service ID

  • Certificate of Naturalization

  • U.S. Access Card

  • U.S. Permanent Resident Card

  • Matricula Consular (Mexico)

  • NEXUS (Canada)

  • U.S. University ID Card

Acceptable Address ID: An acceptable secondary form of identification, traceable to the bearer, is required to verify the validity of the address provided when you apply or request certain products and services. The following acceptable non-photo forms of current identification can be used:​
 

  • U.S. State/Territory/Tribal Driver’s or Nondriver’s ID Card (*If used as a Photo ID, it cannot be reused as an Address ID. You must select a different type of identification for each purpose.)

  • Home or Vehicle Insurance Policy

  • Vehicle Registration Card

  • Current Lease

  • Mortgage or Deed of Trust

  • Voter Card

â–  Note: Utility bill, Social Security cards, credit cards, health insurance card, and birth-certificates are not acceptable forms of ID.

Each individual and/or business owner (with up to 5 total names*) receiving mail addressed to them or their business will need to fill out this USPS FORM 1583  have it signed by a notary public and then email us along with copies of identification that were used.
